Job Description:
As a Human Services Professional at Lutheran Social Services of
Wisconsin and Upper Michigan (LSS), you will play a crucial role in
delivering compassionate and effective support to our clients. LSS
is dedicated to enhancing lives through a variety of programs such
as child and family services, addiction support, community living
assistance, and disaster response. In this role, you will
collaborate with a committed team to address and meet the diverse
needs of the communities we serve, fostering a positive impact and
advocating for the most vulnerable. Assess client needs and provide
direct support in areas including mental health counseling, crisis
intervention, and community resource coordination. Develop and
implement care plans in collaboration with other professionals and
organizations to address complex client issues. Monitor and report
on client progress while maintaining confidential records in
compliance with state regulations and agency policies. Participate
in community outreach and educational programs to enhance program
effectiveness and elevate community awareness. Regular feedback to
supervisors about program implementations and any necessary
adjustments needed to improve client outcomes. Strong interpersonal
and communication skills Experience in crisis management and
intervention Ability to navigate various social services systems
and resources Competency in documentation and record keeping
Experience with case management software and tools Medical
Insurance Dental Insurance Vision Insurance Certified Social Worker
(CSW) or similar certification preferred. Valid driver’s license
and reliable transportation.
