Our mission with each community is to support their success by providing strong leadership, corporate support, and the resources and tools to realize their goals. You'll need to come on-site once a week and anchor the three buildings in the area.Position: Registered DietitianPrimary Responsibilities:Conduct routine visits to assigned facilities, adjusting the frequency based on the complexity of nutritional programs and facility size.Assess the nutritional status of residents, including:Evaluating lab values, dietary intake patterns, weight trends, and food/drug interactions.Assessing dietary compliance, resident independence, and overall well-being.Perform initial and ongoing assessments for new admissions and residents at nutritional risk.Document nutritional assessments and interventions in resident charts and care plans.Review and confirm diet orders, ensuring changes are properly justified and documented.Collaborate closely with facility teams, including Administrators, Food Service Directors, and Nursing Services, to address nutritional concerns.Work alongside interdisciplinary teams (e.g., Speech and Occupational Therapists) to support resident nutrition goals.Provide individualized diet instructions and education to residents, tailoring recommendations to their specific needs.Ensure compliance with state and federal regulations for long-term care facilities and food service programs.Qualifications:Bachelor's Degree in Dietetics, Nutrition, or a related field preferred.Registered Dietitian and hold Licensure in the state of MN is required.Okay to work once a week onsiteKnowledge of state and federal regulations for long-term care facilities and the food service industry.Proven experience in long-term care (preferred).Strong communication and collaboration skills, with the ability to work effectively across teams.Keywords: Registered Dietitian, Clinical Nutrition, Medical Nutrition Therapy, Geriatric Nutrition, Dietitian Long-Term Care, Nutritionist Nursing Home, Senior Care Nutrition, Skilled Nursing Facility Dietitian, Elderly Nutrition Care, SNF Nutrition Services#INDROCHLEAD
