Job DescriptionAdditional Information: We are seeking an individual with a background or licensure in any of the following areas: RN, LPN, social work, or general long term care/ healthcare experience to serve as the Admissions Coordinator for Madonna Towers. The title and compensation for this position will be dependent upon the license and qualifications of the person hired. This position will offer a $2500 Hire on Bonus, a Monday-Friday schedule and access to the Public Service Loan Forgiveness program. Overview: The Case Manager-LPN is responsible for facilitating resident's admissions to and from the community and ensuring a seamless transition. Responsibilities: Enters admissions documents in the system, communicates payment responsibility with perspective resident/power of attorney, and determines Medicare A status with MDS nurses. Develops and communicates with local hospitals/social workers and outlying areas in regards to availability. Works with social services and the resident to determine a discharge plan. Qualifications: Qualifications Required * Current state licensure as a Licensed Practical Nurse (LPN), in good standing. * Graduate of an accredited school of nursing. * CPR certified. * Knowledgeable in Microsoft applications, emails and internet. * Valid driver's license. * Flexible schedule to accommodate admissions. * Ability to read, write and communicate in English.
