This position is open to experienced nurses and new grad
Location: Baldwin 2, Family Medicine, Employee and Community
Patient Care Type: Adult and Pediatric outpatient
Nursing Team Roles: The LPN is an integral member of the care team
and involved in rooming patients for provider visits, assisting
with procedures, providing immunizations and treatments during
office visits, administering care on the LPN nurse calendars in a
variety of aspects, providing simple focused assessments to
patients across the age continuum, providing telehealth based care
through electronic messaging systems in a primary care setting that
is very patient centric.
For new grad nurses: All newly graduated Licensed Practical Nurses
with less than 12 months of practice experience, hired into a
scheduled FTE Registered Nurse position on the Rochester campus of
Employer are automatically enrolled in the Nurse Residency Program
. A separate application or enrollment process is not required. All
nurses new to Employer, regardless of experience, will receive an
extensive orientation program.
The licensed practical nurse (LPN) provides nursing care to
assigned patients under the direct supervision of the registered
nurse (RN) or other assigned supervisor and accepts delegation from
the RN/assigned supervisor in meeting the needs of the
patient/family. The LPN collects data through observation and
communicates information to assist the RN and/or provider in
patient assessment and care planning. The LPN participates as a
member of the health care team and accepts delegation from the
RN/assigned supervisor and provider in meeting needs of the
patient/family. The LPN delivers care in accordance with the
patient care plan, policies and procedures of the organization and
principles of relationship-based care. The LPN possesses excellent
communication skills; is skillful in mentoring and instructing; and
may participate on committees or projects, including quality
Student or graduate of a school of practical nursing or passed LPN
Boards after a defined period in a professional nursing program
(e.g., some states allow RN students to take the LPN board exam
after completing one to two years in the RN program). If graduation
did not occur within the last two years, one year of LPN experience
working in an applicable setting is required.
One year LPN experience within the last five years preferred.
Excellent communication skills (verbal and written).
Experience working in a team environment. Computer skills required,
prior experience with electronic medical record systems preferred.
Ability to work flexible hours, which may include days, evenings,
nights, holidays, weekends, and on-call. Ability to adapt to
unpredictable situations within the work setting. Graduate of an
accredited school of practical nursing preferred.
License or certification
Must obtain prior to start date:
Current LPN license by applicable state requirements.
Midwest: Basic Cardiac Life Support for healthcare providers- BCLS
- certificate of course completion required and must not expire
within 3 months after start date. A scanned copy of the certificate
of completion or transcripts, including your full name, must be
attached to your profile to be considered.

Education, experience and tenure may be considered along with
internal equity when job offers are extended. The minimum pay rate
is $20.95 per hour.
Hours / Pay period
72 hours per 2 week pay period
Monday - Friday; Day & Occasional Evening hours until 7pm, 8-hour
shifts (shift length may vary depending on need) - NOT
Occasional Saturdays (as patient needs dictate) - NOT
