next home care career as a Registered Nurse? Home Health Care
Incorporated has a fantastic opportunity for an RN to perform
admission and start of care visits around the Twin Cities on
weekends. If you have a passion for home care, we encourage you to
apply! Responsibilities: Admission/OASIS Visits: Perform Admission
/ "Start of Care" OASIS Visits throughout an area of the Metro.
Client Evaluation: Evaluate and assess clients for home health
services and create their plan of care. Service Recommendation:
Recommend services from other disciplines that have not yet been
ordered by the physician. Collaboration: Collaborate with clients,
families, physicians, and other members of the home care team.
Qualifications: Licensed as a Registered Nurse in the state of
Minnesota. Driving is required; must have a reliable vehicle with
current insurance and a valid Driver's License. Prior Home Health
Nursing experience. Proficiency with Medicare & OASIS
documentation. Demonstrated ability to work autonomously. Excellent
at planning, organizing, and communicating. Military Occupational
Classification (MOC) Codes: If you're a military member
transitioning to civilian life, we value your service and skills.
