Summary: We’re seeking a Physician (MD/DO) who is energized by the
idea of working in diverse senior settings—from assisted living to
memory care—and building long-standing trust with residents. If

role, this is it. Key Responsibilities: Round at assigned senior
living communities to deliver primary care services Provide medical
oversight, diagnostics, and treatment planning Engage in chronic
care management and reduce unnecessary ER visits Act as a clinical
leader within an interdisciplinary support team Ensure thorough
documentation and follow-up through EMR Qualifications: Active
Minnesota MD or DO license Experience in geriatric medicine,
internal medicine, or primary care Ability to work independently
while collaborating with facility teams Valid driver’s license and
