Case Manager – Home Health Specialist Company: Home Health Care,
Inc. Location: Greater Twin Cities Metro Area Schedule: Full-Time
(Monday to Friday, 8:00 AM – 5:00 PM), 1 weekend/month, 1 on-call
shift/week Compensation: $85,000 – $130,000 annually $6,000 in
bonuses during first year of employment Successful applicants will

Lead comprehensive nursing assessments and individualized care
plans Conduct OASIS visits and coordinate services across care
settings Collaborate with physicians and providers to maintain
clinical continuity Deliver education, emotional support, and
transition planning for families Ensure accurate documentation and
regulatory compliance Qualifications: MN RN license in good
standing 3 years of clinical RN experience; home health preferred
Strong documentation, care planning, and assessment skills Reliable
