Health Rehab Worker FLSA Status: Non-Exempt, Part Time (.5FTE)
Purpose: The Mental Health Rehab Worker (MHRW) in the Community
Support Program setting provides individualized, recovery-oriented
support to adults living with serious mental illness and/or
co-occurring substance use disorders. This role focuses on
empowering individuals to build skills for independent living,
manage symptoms effectively, and connect with community resources.
The MHRW promotes wellness, stability, and self-sufficiency through
education, coaching, and advocacy, while ensuring a safe and
supportive environment. Key Responsibilities: Duties and
Responsibilities: Documentation and Compliance Complete progress
summaries and update treatment plans as needed. Document member
hours and attendance in the electronic health record system.
Maintain confidentiality and adhere to HIPAA regulations. Attend
team meetings and provide shift or service updates as required.
Collaboration and Coordination Provide clients with information for
community resources, including housing, employment, and
transportation. Engage with community providers, family members,
and other support systems as they may visit members at the program.
Communicate promptly with supervisors regarding urgent concerns or
crises. Promote an inclusive, diverse, culturally competent, and
respectful work environment. Support and exhibit Guild values of
empowerment, dignity, collaboration, innovation, and perseverance.
Skill Development and Support Teach and coach clients on daily
living skills such as hygiene, nutrition, budgeting, and use of
public transportation. Provide education on symptom management and
coping strategies. Facilitate individual or group sessions focused
on recovery and wellness. Safety and Crisis Intervention Monitor
client well-being and respond to crisis situations following
established protocols. Ensure a safe environment by addressing
hazards and following emergency procedures. Other duties as
assigned. Required Skills/Abilities: Ability to teach and model
independent living skills. Knowledge of mental illness and recovery
principles. Ability to intervene in crisis situations and promote
stabilization. Proficiency in documentation and use of technology
for service delivery. Ability to develop and maintain receptive,
open, and approachable lines of communication with clients and
staff, and provide a friendly environment for visitors. Strong
written and verbal communication skills, including composing,
proofreading, and editing. Presentation and meeting facilitation
skills. Ability to work as a proactive and positive member of a
team to deliver quality services. Ability to maintain
confidentiality of organization, personnel information, and client
data. Ability to use sound judgment and maintain healthy
professional boundaries. Analytical and problem-solving skills.
Ability to meet deadlines and set priorities. Proficiency with a
variety of web-based software and databases, including Microsoft
Office Suite. Working Conditions: Work is primarily at the Guild
Community Support Program member center. Some activities may be
required in the community. Travel may be required to other program
sites. Prolonged periods at a desk, working on a computer. Walking
and standing continuously for hours at a time. Manual dexterity for
tasks such as taking notes, using technological devices for
documentation, or assisting clients with certain activities if
needed. Must be ambulatory and able to negotiate stairways. The
ability to lift and carry heavy weights up to 25 pounds. Education
and Experience: Required Must meet all of the following criteria to
be a Mental Health Rehabilitation Worker in Minnesota (Minn. Stat.
245I:04): Have a high school diploma or equivalent Have
successfully completed the training required under Minnesota
Statute Section 2451.05, which includes 30 hours of training on the
topics of mental illness, client recovery and resiliency, mental
health de-escalation techniques, co-occurring mental illness and
substance use disorders, and psychotropic medications and
medication side effects. Meet 1 of the following qualification
requirements under Minn Stat 245I:05): Be fluent in a non-English
language or competent in the culture of an ethnic group served.
Have an associate of arts degree. Have two years of full-time
postsecondary education or 15 semester hours in behavioral sciences
or related fields. Be a registered nurse. Have, within the previous
ten years, three years of personal life experience with mental
illness. Have, within the previous ten years, three years of life
experience as a primary caregiver to an adult with mental illness,
traumatic brain injury, substance use disorder, or developmental
disability. Have, within the previous ten years, 2,000 hours of
work experience providing health and human services to individuals.
Preferred Associate’s degree in human services or one of the
behavioral sciences 1 year of experience (2,000 hours) working with
adults with mental illness Additional Requirements: Must not be
actively receiving services from Guild and must not have received
services within the last two years. A valid Class ‘D’ Driver’s
License, reliable vehicle, satisfactory driving record and

$22.00 to $25.00/hourly. The range reflects the minimum and maximum
salary for the position. Compensation will depend on the
candidate’s professional experience, skills, and relevant
