Aveanna Healthcare is seeking a dedicated Home Health Physical Therapist to join our team in a full-time capacity, with a $10,000 sign-on bonus to welcome you. In this role, you will be integral in helping our patients achieve their highest level of functionality and independence through direct care provided at their homes. Our focus is on setting patient-specific goals and employing innovative therapy techniques designed for each patient's unique situation, ensuring personalized and effective care. Assess patient health conditions and work with other healthcare professionals to formulate personalized care plans. Implement physical therapy interventions and continuously evaluate the effectiveness of these interventions, making adjustments as necessary. Educate patients and families on treatments and in-home therapies to encourage continued care and exercises. Maintain accurate patient records and report on progress to supervising healthcare provider. Manage a caseload with attention to quality standards and productivity. Guide and instruct home health aides or assistants in specific physical therapy techniques. Ability to handle a diverse caseload effectively Strong skills in physical therapy assessment and treatment Excellent communication and interpersonal skills Proficiency in documenting and tracking treatment plans Medical Insurance Dental Insurance Vision Insurance 401k Valid Physical Therapy license in the practicing state CPR certification recommended
