Physician Practice Setting: Outpatient and Inpatient
Hospital/Surgery Center Privileges: Required Admissions: Required

Level 2 Pediatric Level: Level 0 Patients Per Shift: 15-18 Bed In

Patients Seen: 0-5 Call Back: Percentage and details to be

Pediatric, and Neonate patients Advanced Endoscopic Techniques and
Laparoscopic Surgeries Ureteroscopy, Nephrectomy, and Prostatectomy
Robotic Surgery and Incontinence Procedures Extracorporeal Shock
Wave Lithotripsy (ESWL) and Renal Transplant Surgery Additional
Details: Coverage needed for specific dates and ongoing needs
Potential for 20-30 procedures per week Handing off to permanent
providers Utilization of "real-time" Mobile Time Clock required If
