Job Description:

All therapy/rehab traveler start dates will be booked 6 weeks out to provide time to process Marshfield credentialing and onboarding requirements. All travelers will be granted temporary privileges through the Marshfield credentialing team. Marshfield's HR will reach out to the traveler directly alongside our Vaya compliance team throughout the onboarding process to collect these requirements. PT, SLP, & OT placements will be required to submit the following requirements within 1 week of signing - These documents are required for the credentialing application and failure to complete these timely could result in a pushed start date - DOB, SSN, resume, transcript or diploma, education verification, & minimum of 3 references 2 peers and 1 department chair (need name and email address) 13 weeks 20HPW, Mon 08:00-15:30, Tues 08:00-15:30, and Wed 08:00-14:30 Mon-Tues 7 hrs and Wed 6 hrs 30 min lunch required Every third weekend 4 hrs every 3rd Saturday (subject to change) 1st time travelers accepted Required: 1yr exp, BLS, WI State License (pending licenses accepted), and Bachelor's, Master's or Doctorate degree in physical therapy
