would see patients in the Southeast Minnesota geography including
the towns of Rochester and surrounding areas. Mileage is
reimbursed. The Hospice Aide (HA) provides personal care,
homemaking services and other supportive services independently to
persons who are receiving Hospice care. All such services will be
provided in accordance with an individualized, comprehensive
patient care plan and under the supervision of a Registered Nurse
in compliance with the agency's policies and standards of practice.
The Hospice Aide will provide feedback to the Hospice
Interdisciplinary Group (IDG) regarding patient status and
participate in the patient care planning process. Examples of tasks
the Hospice Aide may perform include, but are not limited to,
assisting with ambulation, meal preparation, range of motion and
vital sign measurement and documentation. The Hospice Aide will
provide patient and family centered care in a culturally sensitive
manner. Qualifications 18 years old. Completion of 12 college
credits -OR- high school diploma/GED. Successful completion of a
state approved licensure program for Home Health Aide or Nursing
Assistant Training Program or a student enrolled in an approved
health/social science program who has completed a Nursing Assistant
Training Program. If a Home Health Aide, Hospice Aide, or
equivalent has not provided services for compensation for a
consecutive 24-month period, the individual is considered to not
have completed a training or competence program and must complete a
program before providing services. Applicant must provide
verification of satisfactory completion of program. Additional
Qualifications Current listing on the Nursing Assistant Registry
with the Minnesota Department of Health preferred. Certified
Hospice and Palliative Nursing Assistant (CHPNA) preferred but not
required. Experience with end-of-life and/or the geriatric
population preferred. Must possess clear written and verbal
communication and excellent interpersonal skills; must be able to
work independently. Must foster a team atmosphere by maintaining an
open communication with the interdisciplinary team; must have the
ability to recognize any problems/concerns that should be brought
to the attention of proper Hospice personnel. Ability to read,
write, and carry out directions. Ability to assemble, maintain and
record data in an accurate record. Must provide personal mode of
transportation to patient's home. Valid driver's license and proof
of auto insurance required. Maintains current Basic Life Support
for Health Care Providers from one of the following programs:
American Heart Association or American Red Cross. If a candidate
does not currently have their Basic Life Support for Health Care
Providers from one of the following programs: American Heart
Association or American Red Cross the Basic Life Support,
certification will be included as part of the orientation program.
Exemption Status Nonexempt Compensation Detail $18.14 - $24.67/hour
