Are you a passionate advanced practice provider ready to serve the

Minnesota Community Care. We are looking for experienced pediatric,
family or adult nurse practitioner or physician assistant to help
in our School Based Clinics. This role is in a school clinic, and
the scheduled hours are during the 42-week school year. There might
be opportunity for hours during school breaks, based on
availability.

manner; listening to and answering questions from patients; and
clearly explaining diagnoses and treatments
- Provide whole person care by using appropriate referrals to the
social worker, psychologist, psychiatrist, integrating social and
structural determinants of health, including historical trauma,
structural racism and biological, social, psychological and

Work Environment and Physical Demands
This job operates in a healthcare setting. This role may
occasionally come into contact with patients who may have
contagious illnesses. This position is very active and requires
standing, and/or walking, bending, kneeling, and stooping. The
employee may need to lift and/or move items over 20 pounds.

Qualifications
Advance Practice Registered Nurse
- Practice, Pediatric or Adult Nurse Practitioner or Certified
Nurse Midwifery programs.)
- Completion/Certification of an approved Nurse Practitioner
program and nationally approved Nurse Practitioner Certification
Board is required (i.e. Adult Nurse Practitioner; Family Nurse
Practitioner; Women's Health Nurse Practitioner; Pediatric Nurse
Practitioner.)
- Minnesota Board of Nursing RN licensure, as well as Advance
Practice Registered Nurse, for Advanced Practice Nurses.
- Prescription writing privileges
- Responsibility for continuing education credits to maintain
licensure.
- Master's degree, or higher, is required for practice as an
Advance Practice Registered Nurse (including Family, Pediatric and
Adult)
- Basic Life Support certification.
- DEA certification
- Bilingual verbal and written skills in language(s) of primary
target population(s) required; Spanish and/or Hmong -
PREFERRED.
- Proof of COVID Vaccination Required, Religious and Medical
exemption will be considered Physician Assistants
- Master's degree, or higher, is required for practice as a
Physician Assistant Studies.
- Completion of an approved study program is required for
practice as a Physician Assistant. With Certification through
NCCPA. Licensed to practice in Minnesota as a Physicians Assistant.
Completion of an approved study program is required for practice as
a Women's Health Nurse Practitioner or Physician Assistant.
- Prescription writing privileges
- Responsibility for continuing education credits to maintain
licensure.
- Basic Life Support certification.
- DEA certification
- Bilingual verbal and written skills in language(s) of primary
target population(s) required; Spanish and/or Hmong -
PREFERRED.

- Minimum one year Advance Practice Registered Nurse/Physician
Assistant experience with school ages youth and adolescent health
and/or GYN preferred.
