Primary Care in the Rochester and surrounding region is seeking a
Nurse Practitioner or Physician Assistant to work as part of a
multidisciplinary care team to provide care to those living within
our community through the Senior Service Homebound Programs. Within
in this position the NP/PA will work collaboratively with internal
medicine physicians to providing provide holistic care including
acute, chronic, preventive and palliative care services to patients
in private homes and senior living centers. The provision of care
includes but is not limited to comprehensive evaluation, ordering
and interpreting diagnostic tests, prescription of pharmacologic
agents and treatments and non-pharmacologic therapies, and
counseling patients. Ideal candidates have experience in Palliative
Care, Primary care and/or Geriatrics.
Masters prepared graduate of an accredited school of nursing and
Nurse Practitioner program. Current Minnesota RN license and
certification as an ACNP, ANP, GNP or FNP. (DNP is preferred)
Or graduate of an accredited Physician Assistant program with
current PA certification by NCCPA and registration as a PA by the
MN state Board of Medical Practice.
DEA registration and BLS certification required.
Experience in geriatrics, primary care management or home health
